Crossroads Apothecary Supplement Access Program

The Crossroads Apothecary Supplement Access Program is an innovative new program designed to help reduce your expense for the supplements that are prescribed by your Crossroads Healthcare Practitioner.  This unique program allows you to continue to buy your high quality nutritional supplements at the manufacturer's advertised wholesale cost.  If you are spending an average of $125 or more per month on your prescribed supplements, you may benefit from this program.  Your Crossroads Apothecary representative can help determine if this program is right for you.

 

Annual Membership Payment Options:

 

1)      The prepaid yearly fee is $540 ($45 per month).  Additional family members would be $270.

2)      The bi-yearly fee is $330 every six months ($55 per month or $660 per year). To include additional family members, please add $135 each, for every six months.  The bi-yearly fee will be charged to your credit card at the six month interval, unless other arrangements have been made.

3)      The fee for our First Line Therapy 90-day Program is $135.  You may switch to the yearly Supplement Access Program at the time of your 90-day expiration, at a prorated fee.  Additional family members are required to pay $135 membership fee to join.  There are no additional family discounts on the First Line Therapy 90-day Program.

Important: If you should choose to discontinue the First Line Therapy program, your Supplement Access Program will also be immediately terminated without refund.

Thank you for your interest in our Supplement Access Program.  The Crossroads Apothecary strives to be a partner in your quest for optimal health.  We hope this program will aid and support your compliance with your customized nutritional protocol prescribed by your Crossroads Healthcare Practitioner.  We also encourage you to schedule quarterly evaluations, so that your road map for health optimization can be updated as needed.  If you have any questions about your membership please contact your Crossroads Apothecary representative at:

 

Terms and Conditions:

Your prepaid membership with the Crossroads Apothecary Supplement Access Program allows you access to nutritional supplements sold by the Crossroads Apothecary at the manufacturer's suggested wholesale price and to include any additional fees incurred. The patient must be seen by his/her doctor every four months for follow-ups and possible protocol adjustments.  The patient is required to bring an official copy of their doctor’s recommendations with them in order to purchase at wholesale prices.  Other discounts may not be combined with this program. We may limit the amount of a particular item purchased - per visit - at our discretion. This program applies only to the patient whose name appears on the contract and does not extend to family and friends.  This contract is non-transferable and may be canceled at any time.

A cancellation fee equal to 10% of your total contract will apply.  Refunds will be given only for the months remaining on this yearly contract after the cancellation request is received in writing.  No refunds will be given for the months where no purchases were made.  This program applies to purchases made after the contract goes into effect and is not retroactive.  Shipping charges are applicable for all mail orders with a minimum fee of $8.50.  Additional shipping charges may be applicable for special orders.

 

Membership program and fees are subject to change at the end of the yearly contract.  Renewal of your Supplement Access Program requires submission of a new contract.  Bi-yearly fees, if applicable, will automatically be billed at the appropriate six month interval.  Non-payment of the bi-yearly fee will be viewed as a cancellation request and the 10% cancellation fee will apply.  The Crossroads Apothecary reserves the right to terminate this program for any patient if it is determined that the program is being abused.
